Reports To: Director of Elderly Services or his designee.

Objectives

Responsible for the Scheduling of client services and implementation of the Torrington Chore Program, Assisted Medical Transportation Rides and assist with Data tracking and Collection Title IIIC, RITS, State of CT Municipal Transportation Grants for Seniors. Track UOS (Units of Service), billable hours, income, Employee Mileage, and expenses for all applicable programs.

 

Essential Functions

  • Schedule and assign services for clients, including transportation & chore services.
  • Prepare daily route sheets for drivers and weekly client work verification forms for chore workers.
  • Answer Phone, provide assistance to daily Office Function, Work with Elderly Services Software Program (Serve Tracker) and City Software (New World)
  • Coordinate with area convalescent homes, medical offices, NWCT Transit District, RITS reginal Operators to schedule transportation for patients.
  • Schedule van maintenance and lift service for all transportation vans with City Maintenance Garage and preventative Maintenance provider.
  • Assist completing client information forms and assist to submit monthly MIS to WCAAA for UOS reimbursement.
  • Assists completion and submission of monthly Financials and Quarterly Reports and Data collection to the WCAAA, Northwest Council of Governments Reginal Transportation (NWCOG) RITS, State of CT DSS and DOT
  • Participate in WCAAA program audits Data verifications and Collection.
  • Assist to collect and submit monthly statistical reports and billing to WCAAA, CCCI for the Chore Program, Title IIIC and NWCT Transit District for Municipal Transportation Grant, RITS for Seniors and Disabled
  • Attending regional transportation meetings and WCAAA required meetings and training when needed.
  • Assist with monitoring and organization of employee training requirements.
  • Assist to Compile and Verify payroll hours and Monthly Mileage sheets.
  • Assist and prepare weekly bank deposits and verifications.
  • Assist with annual Grant applications, DOT applications, RITS applications and other Grant applications as needed.
  • Assist to Plan and Organize fundraising events to meet grant match requirements.
  • Refer appropriate clients to area supportive & ST of CT Protective Services when necessary.
  • Knowledge of Desktop, Cell Phones, Tablets, Internet Portal accessibility, Knowledge of WORD, Excel, PowerPoint needed. Training available.
  • Assist to Submit documentation for the State of CT Certification to the Department of Consumer Protection, Accreditation Credentialing as required.

 

QUALIFICATIONS

Applicant must possess a desire to work in social service and primarily with the over 60 population with the desire to train and learn on the job in a flexible service environment. Desired experience with work Scheduling, event planning, computer skills, understanding basic Budgeting and Local Government a plus. Social Service Experience to include but not required, graduation from college with a BS degree preferably but not required in Human Services, Social Services, Gerontology or related field plus experience in office multitasking environment. Able to sit at a desk for several hours per day, Lift 25 pounds and ability to interact in person and on the phone with elderly clients.
