Health Assessment Program

A Partnership between
The City of Torrington
and
Charlotte Hungerford Hospital

nurse.JPGThe Health Assessment Program provides comprehensive health care services to residents of Torrington, Litchfield, Goshen and Harwinton who are age 60 or older.

The purpose of the program is to provide health promotion and prevention services, early detection of illness, education and counseling to promote a healthy life style.

Services provided include:

Physicals: suggested donation $25.00

Vision and Hearing Screening
Blood Pressure, Pulse, Respirations, Height and Weight
Health History
Physical Examination
Pap/Pelvic/Breast/Exam
Prostate Exam
Health Education
Lab Services:  Urinalysis, stools for blood.  Specific blood  work is individualized by diagnosis.
Electrocardiograms:  as indicated

Ear Assessment and Irrigation Clinics:
The 3rd Wednesday of the month by appointment.
Suggested Donation:  Assessment -$ 5.00  Irrigation   - $20.00  

Where:  Sullivan Senior Center, Health Office Bldg. B

Scheduling:  Call 489-2211 Monday - Friday between 9:00a.m. to 3:00 p.m.  for an appointment.

Lab Fees:  Coverage depends on your insurance carrier.

Preparation:  Fasting after midnight for early morning appointments only.  No fasting required for later appointments.  Urine specimens will be collected as part of the exam.

Home visits are designed to evaluate an individuals health and self care needs . The purpose of the home visit is to determine the level of health and/or social services necessary to promote the individuals health, safety and independence.  

Office visits are provided for health education and counseling. Weekly blood pressure screening is available on Thursdays from 10:00 a.m.- 11:30 a.m.

Kathy Grimaud, APRN, MSN, nurse practitioner is responsible for the health assessment, intervention and referral services.

Chris Sherwood assists the nurse practitioner and is a certified medical assistant and phlebotomist.

Deborah Reardon, LPN, is available on Thursday mornings for blood pressure screening.

John Romano, MD, provides medical consultation.

The Health Assessment Program is funded  through  a grant from the Western Connecticut Area Agency on Aging, made possible through the Older Americans Act; the City of Torrington and the Charlotte Hungerford Hospital and client donations.
